- Develop and implement comprehensive social services programs that address the diverse needs of our residents
- Provide leadership to the Social Services team, ensuring the delivery of high-quality, person-centered care
- Collaborate with interdisciplinary teams to create individualized care plans that enhance the overall well-being of residents
- Act as a liaison between residents, families, and community resources to support a seamless transition into our facility and assist with discharge planning
- Advocate for residents' rights, ensuring they receive the highest standard of care and maintaining their dignity and independence

- Master’s degree in social work or related field
- Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred
- Minimum of 4 years in social services, with at least 2 of those years in a leadership role in a long-term care setting
- Strong understanding of the aging process, psychosocial needs of the elderly, and the impact of chronic illness
- Ability to collaborate effectively with a diverse team and build positive relationships with residents and their families
