      As the Director of Social Services, your role is to work with the interdisciplinary team and residents to ensure that the medically related emotional and social needs of our residents are met and maintained. Work with residents and their families to develop individual care plans including comprehensive discharge planning. Coordinates with the MDS team to complete Social Services portions of the MDS and oversees the grievance process as the acting Grievance Official.

      Who are you:

      • Bachelor’s degree in social work, or equivalent combination of education in related field and experience preferred.
      • Minimum 5 years of experience (and passion) in social work in a long-term care setting with supervisor experience preferred.
      • Experience in working directly with families.
      • Excellent customer service and people skills.
      • Demonstrated ability to use discretion in working with sensitive and/or confidential information.
      • Must pass background check and obtain Arizona Fingerprint Clearance Level 1 (we can help with the card).
      • Excellent communication skills – written and verbal.
