      POSITION PORPOSE

      • The Social Care Director is a state-wide position, supporting nine hospitals. They are a member of the Community Health and Well-Being for the department. Seeking an experienced CHWB professional with a relevant bachelor’s degree.

      • The selected candidate is Responsible for the oversight and coordination of the operations of Social Care including but not limited to the Community Health Worker (CHW) program, healthy social care programs, the Community Resource Directory, mobile medical units, and collaboration with Safety Net Health Centers.

      • Responsible for successfully identifying and implementing programs and services to support vulnerable populations by integrating social and clinical care through strategic collaboration with Medical Group practices, acute care colleagues, and Clinically Integrated Networks.

      • Full-Time position supporting a statewide department and candidates can be based in West Michigan or SE Michigan.

      WHAT YOU WILL DO

      • The Social Care Director Plays an integral role in successfully coordinating activities of Community Health Workers and social care program facilitators who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, Ensures long-term sustainability of the CHW, social care programs and additional Social Care initiatives.

      • Develops and implements a plan to advance Social Care that includes developing a robust Community Health Worker hub, integrating social and clinical care, and implementing programs and services to support vulnerable populations.

      • Supports CHW, social care programs, and daily activities, including hiring and training of CHWs and program facilitators, ongoing evaluation of CHW and program facilitators’ skills, providing regular feedback and communication, and ensuring compliance with local policies, procedures, roles, and responsibilities.

      • Conducts quarterly chart audits to ensure timely and accurate documentation and delivery of social care services and to identify opportunities for improvement.

      • Assesses and promotes opportunities to integrate CHWs and other social care services into service delivery within the Health System.

      • Ensures that the CHW and social care programs are appropriately integrated into all parts of the Health System, securing necessary resources for the CHWs and program facilitators, planning and facilitating partnerships in the community and within the RHM, facilitating coordination between the CHW and social care programs and other departments, and helping to maintain connection between the CHWs and providers, care managers and other contacts.

      • Develops and administers budgets for programs, equipment, and support services.

      • Markets all social care services including: the Community Resource Directory and social care program activities and implements strategies to increase utilization and expand referral networks.

      MINIUMUM QUALIFICATIONS

      • Must possess a comprehensive knowledge of health disparities, social influencers of health, uncompensated and undercompensated care, community benefit, community and coalition building, and population health management.

      • EDUCATION: Bachelor's degree in Health Care Administration, Public Health, Community Health, Social Work or a related field. Master’s Degree is preferred.

      • EXPERIENCE: five or more years of strategic planning and fiscal management experience in community health and well-being area in a complex organization.

      • Ability to effectively manage and facilitate colleagues within assigned unit/department through strong leadership, communication, and negotiation skills. Ability to influence results, garner support and tactfully manage complex relationships and influences within and across the organization. Ability to read the subtle nuances of situations and react/plan accordingly.
