Job Description

- Responsible for initiating, engaging and maintaining contact with the injured/absent employees on a regular basis
- Provide support to employee’s manager/supervisor and client’s Human Resources (HR) team regarding case management, disability and accommodation
- Able to effectively and collaboratively resolve conflict
- Interpret and educate the client’s managers on a variety of regulatory and compliance requirements to ensure that case management practices are applied in accordance with legislation, regulation and our clients’ policies.
- Make referrals to appropriate resources such as EFAP provider, the client’s Occupational Health Physician, or Ergonomic Consultant, as required
- Effectively manage a fluctuating case load
- Collaborate and liaise with other members of WSIB and the client’s Health & Safety team
- Act as liaison, mentor and coach regarding disability claims to both internal and external partners
- Develop effective and sustainable return to work plans
- In collaboration with the client’s managers and human resources business partners, regularly monitor and adjust the progress of all return to work cases to resolve emerging issues
- Develop effective relations to collaborate with the employee as well as the client's human resources, management, union and relevant health care professionals to implement return to work and /or modified work programs
- Provide education and training to the client’s managers and staff regarding the disability case management processes
- Maintain and respect confidential information; objectively and accurately document case history, strategy and case management plan
- Accountable for regularly gathering, maintaining and reporting statistics regarding for STD and LTD, and be able to proactively identify trends.
- Certified Disability Management Professional or Certified Return to Work Coordinator credential is required. If this is not a current credential, it is expected that the candidate is working toward completion in the next 12 months.
- At least 2 years of experience in disability case management.
- Post-secondary degree in a health related field required.
- Demonstrated expertise and experience with return to work planning, long term disability contracts, collective agreements, and attendance and accommodation programs in a large and diverse organizational environment.
- Excellent interpersonal skills required.
- Working knowledge of relevant legislation (e.g., Ontario Human Rights Code, Occupational Health and Safety Act).
- Excellent communication, organization and presentation skills.
- Excellent computer skills (Word, Excel, HRIS); able to create and generate reports as required.
- Superior assessment and problem solving skills.
- Knowledge of the Workplace Safety and Insurance Act is an asset.

Position Summary:
Reporting to IHS and the client as applicable, the Disability Case Manager (DCM) is a highly valued member of our client’s team and is responsible for the proactive and expert management of short-term disability (STD) and Workplace Safety and Insurance Board (WSIB) claims as well as bridging long-term disability (LTD) files from the STD. The DCM collaborates with managers and other key partners in return to work support.
As a member of a dynamic interdisciplinary team of professionals, your expertise in this field will enable you to act as an important contributor to discussions regarding workplace disability related issues.

The Disability Case Manager will be working at THP client sites, in Mississauga and West Toronto as directed by THP, in its sole discretion. There is some opportunity for flexible work environments (approximately 70% in office/30% home). The primary work space will be working from various THP client sites, as directed by THP, in its sole discretion.
