      Job summary

      As a Discharge Coordinator working with the Discharge Team at Pilgrim Hospital, you will be allocated to an area and will be responsible for assisting in the discharge planning for our complex patients. This is a patient facing role. You will liaise with the wider MDT, both internal and external, including adult care, community health, reablement services, therapists, and others, to plan a timely discharge.

      The role can be extremely challenging and we are looking for persons who are enthusiastic, work smart to solve problems and are able to work independently as well as part of a team.

      Main duties of the job

      It is the Hub's responsibility to hold regular MDT's to ensure that there are minimal delays built into our patients length of stay.

      We provide a 7 day service and applicants must be aware of the need to work within a rota to cover, this service.

      An understanding of health and social care provision in the community would be an advantage but not essential as we will support education and training. We are looking for team members who will have a real desire to make a difference for patients and their families.

      Good keyboard and IT skills, including knowledge of Microsoft Outlook, Word and Excel are also essential. You will be expected to manage your own case load with the support of the Registered work forceand discharge lead.

      Full training will be given to the successful applicants, supported by the Discharge Team.

      The successful candidates will rotationally work in the Integrated Discharge HUB and the clinical environment. This will be working with system partners to identify patient discharge pathways.
