      The AMU unit at Poole hospital is pleased to announce the opportunity for a dynamic, organised and proactive Band 3 Discharge co-ordinator to come and join our team.

      As the successful post holder you will be expected to work closely with the nursing staff and wider MDT including Discharge Team, Social Services, Care Agencies and Care/Nursing Homes to facilitate the safe and effective discharge of our patients. You will gain experience in managing complex discharges, safeguarding, dealing with challenging situations and communicating with outside agencies.

      The post comes with full on the job training with an induction to the hospital and the role included.

Any other changes unrelated to this, will be handled in line with Trust/National terms and conditions.
