Job Description

- Develops a discharge plan and uses person centered planning to engage each participant in his or her individual recovery journey.
- Documents information and services provided in the electronic health record system to assure continuity of services for DWIHN clients.
- Meets with guests on a daily basis to assist with navigating the discharge process.
- Partners with the participant to obtain collateral information from the person’s support team such as family members, friends, case manager and other area programs.
- Obtains details of Advance Directives such as Mental Healthcare Power of Attorney and WRAP crisis plans to design and plan care.
- Provides information to each person served about their legal status, their service options, and the process of care such as next and subsequent steps.
- Coordinates with guests, peer support specialists and other care center staff to develop aftercare plans.
- Refers and connects the guest with aftercare appointments.
- Directs the discharge process and completes electronic discharge of guests.
- Observes and supports floor and workflow as a Qualified professional.
- Maintains the safety of guests and staff.
- Creates meaningful contacts with guests.
- Connects with community resources to promote development of referral relationships.
- Assists in following up with guests after discharge to ensure they are appropriately connected with supports.
- Maintains and observes all HIPAA regulations.
- Provides timely, quality, and accurate documentation in accordance with program standards.
- Participates as a team member to provide a safe recovery environment.
- Utilizes infection control and other safety standards.
- Contributes to the cleanliness of the workplace.
- Serves as an advocate for the needs and rights of every person.
- Verifies that all required documentation is completed prior to discharge.
- Arranges for transportation services.
- Notifies patients and families of upcoming discharge dates and times.
- Follows up with patients after discharge to ensure a smooth transition of care.
- Maintains accurate records of all discharge activities.
- Collaborates with interdisciplinary team members to develop individualized discharge plans.
- Educates patients and families on post-discharge care needs and available resources.
- Monitors compliance with regulatory requirements related to discharge planning.
- Participates in quality improvement initiatives related to discharge planning.
- Performs related duties as assigned.

Under the general supervision of the Site Director, the Discharge Coordinator assists with discharge planning and coordination of care for crisis care clients.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
KNOWLEDGE, SKILLS AND ABILITIES (KSA’S):
REQUIRED EDUCATION:
A Bachelor’s Degree from a recognized college or university in the Human Services, the Social Services, Nursing, Public Health, Public Administration, Healthcare Administration, Health Management, Psychology, Counseling, Social Work, or a related field.
REQUIRED EXPERIENCE:
One (1) year of full-time paid professional experience working in a human service, social service, mental health or behavioral health setting.
REQUIRED LICENSE(S).
A valid State of Michigan Driver’s License with a safe and acceptable driving record.
SPECIAL CONDITIONS:
Must obtain CPR certification within 6 months of hire.
WORKING CONDITIONS :
This position will work in the DWIHN Crisis Care Center. This position will be required to work with persons who may suffer from mental health or substance abuse crises.
Crisis Care Operations is a 24/7 unit. Candidates must be able to work a flexible schedule.
