- Conduct rounds with physicians to meet with patients to discuss placement orders
- Assist patients with a specific plan for medication management
- Include patient in discharge planning
- Assist patients with the identification of a support network, instructions for future care and follow-up
- Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors
- Bachelor’s degree or a clinical license
NaphCare is hiring an experienced Discharge Planner just like you to join our team at the Pima County Detention Center located in Tucson, AZ .Full Time Day Shifts are available! Pay rates range between $26.00 - $34.00 per hour based on shift, site needs, and experience.

We have opportunities for full-time with great benefits, part-time, PRN, and 13-week temporary contracts available. PRN shifts and hours vary based on coverage needs and will be determined by site leadership and prn employee availability.
