      Job summary

      A rare opportunity has arisen in our friendly and professional team in the seaside town of Swanage for a Specialist District Nurse Practitioner.

      We are an integrated team across Purbeck and in our rural locality we work very closely with other colleagues and services to get the best outcome for our patients. There is much focus at the current time on the future of the Integrated Neighbourhood Team, which you will be contributing to the development of.

      You'll need a good understanding of the community healthcare agenda; you'll be using excellent communication, decision making and leadership skills. You'll be supported to provide high quality and critical clinical care, management of team members and caseload management for patients in the community setting. You'll need experience in caseload management and have a track record of being able to prioritise your work whilst advising others on theirs. The role is diverse, challenging and extremely rewarding.

      Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

      Main duties of the job

      The role also involves being a member of the senior team for the locality, co-ordinating care and workloads, managing and leading the community nursing teams with the guidance and support of the District Nurse Locality Lead.

      You must be committed to advance your own clinical knowledge, skills and competence based on current evidence, through advanced educational programmes.

      To assess, plan, implement and evaluate specialist treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team. Plan, implement and review health improvement programmes in a range of settings.

      • Membership of the relevant Professional Body
      • Learning and Assessing in Practice Qualification or equivalent practice assessors training
      • Registered practitioner SPQDN qualification or a strong desire to achieve this.

      • Knowledge and understanding of Trust Strategy relevant to role
      • Evidence of recent professional development in an up to date portfolio

      • Experience at Practitioner Band 5 level with extended roles for community nursing
