Job Description
- Req#: 18189
- Consults and collaborates with medical center staff to coordinate interdepartmental patient services. Develops comprehensive and realistic treatment plans based on patient's needs.
- Provides short term follow-up to ensure linkage and stabilization. Provides advocacy and support to patients to secure treatment and services.
- Engages families, treatment providers, and other parties active in the well-being of the patient whenever possible. Provides technical support, assistance, and education to patients, families, and other patient service providers.
- Assumes the duties of charge person when required.
- Assumes the duties of mobile outreach on-call when required.
- Documents all telephone triage information; clinical assistment data; changes in mental status; and all communications related to patient referrals, transfers, and admissions in the patient record.
- Master's degree in a related field and one year mental health experience in an inpatient setting, screening center or emergency serivce.

- Frequent physical demands include: Standing , Walking , Reaching forward , Keyboard use/repetitive motion Occasional physical demands include: Sitting , Climbing (e.g., stairs or ladders) , Carry objects , Push/Pull , Twisting , Bending , Reaching overhead , Squat/kneel/crawl , Wrist position deviation , Pinching/fine motor activities Continuous physical demands include: Taste or Smell , Talk or Hear Lifting Floor to Waist 40 lbs. Lifting Waist Level and Above 40 lbs.
Responsibilities
Provides Mental Health Screener functions and performs psychosocial assessments including collections of historical data, mental status assessments, and suicide risk assessments.
