Job Description

- Coordinates and organizes projects and meetings for the ECM program;
- Coordinates and facilitates both internal and external meetings;
- Develops and publishes detailed meeting agendas, meeting minutes that meet NCQA standards, and any other necessary documentation;
- Identifies and manages project deliverables, dependencies, and critical path milestones as necessary;
- Supports the successful implementation of projects within timelines for associated department assignments and tasks;
- Provides technical and administrative support for the development of business cases and system reports for projects and/or programs;
- Works with the ECM Administrative Team to develop and refine tactical department and program initiatives;
- Attends project meetings, follows up on assigned tasks, and communicates the status of projects to the ECM Administrative Team.
- Develops and publishes communications to project stakeholders.
- May assist the ECM Administrative Team with reviewing KHS policies and procedures to ensure they continually meet DHCS requirements;
- May assist the ECM Administrative team and other partners with monitoring Key Performance Indicators for the ECM program;
- Collaborates with the ECM Administrative Team, leaders and staff, subcontractors, network providers, and entities included, but not limited to local community-based organizations, local health department, behavioral health and social services, child welfare systems and members in ECM efforts and initiatives;
- Performs miscellaneous job-related duties as assigned;
- Bachelor’s degree in social work, health care, public policy, business, or related field preferred;
- Minimum of two (2) years’ experience as a Senior Support clerk or equivalent experience including strong organizational and planning skills; experience in project planning preferred; any equivalent combination of education and experience.
- Ability to execute quickly and handle multiple projects simultaneously;
- Strong attention to detail;
- Proven track record of successful program coordination;
- Ability to interpret quantitative analyses of operational problems;
- Ability to understand and implement continuous improvement processes;
- Ability to analyze business results and develop programmatic and operational reports.

KHS reasonably expects to pay starting compensation for the position of Enhanced Care Management Program Coordinator in the range of $30.20 – $40.05 hourly.

Definition:
This position reports to the Manager and the Director of the Enhanced Care Management Department. The ECM Program Coordinator will coordinate and lead program planning for the ECM program overall. The ECM program is required through the Department of Health Care Services (DHCS) to contract with program providers in the community. The ECM program currently has 28 program providers and is expected to continue to expand services and contracts. This position will work directly with the ECM Administrative team and the contracted ECM providers to coordinate the overall program. This will include assisting with provider questions, issue resolution, project management, arranging Operational/Quality meetings, organizing agendas, and preparing meeting minutes in accordance with NCQA accreditation standards.
Distinguishing Characteristics:
Partner with the ECM Administrative team and leaders across the organization to support the development of strategies that align with organizational objectives and assist with the implementation of the ECM program throughout Kern County. Partner with ECM program providers to ensure program compliance. Help track and measure any quality improvement strategies for the ECM program overall. Ensure regular communication between the ECM program and program providers.
