Job summary
This Role is a PCN Additional Role and Responsibilities (ARRS) vacancy funded by the PCN to work for Solent GP and Homeless Health Care Practices as an Enhanced Nurse Practitioner. Solent GP Surgery and Homeless Health care provides Primary Care across four locations in Southampton City. Solent GP Surgery is based across 4 sites Portswood, Nicholstown, Adelaide, and Homeless Healthcare surgery, holding a patient list of approximately 22,000 patients.
Homeless Healthcare GP Surgery has been in existence for over 20 years and holds a smaller patient list of approximately 300 to 400 patients. Both GP Surgeries hold an APMS contract via Hampshire and IOW ICB and run TPP S1. Both Surgeries are members of Southampton Central Primary Care Network (PCN), encompassing 56,000 patients and 4 practices in total.
An Enhanced Nurse Practitioner is to be autonomous and working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending a primary care setting. In liaison with the duty GP to run a daily triage and see and treat service as part of the duty team for the day. This will include managing all patients who require an on the day appointment either by telephone or by face-to-face consultation.
Main duties of the job
To provide professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence
To be an integral member of the practice team across all sites to support the development of new and innovative concepts, models, methods, and practices ensuring the delivery of new and improved primary care services enabling the practice to meet the needs of the practice population.
To autonomously practice high quality evidence-based care by utilising knowledge, interpersonal, history taking and assessment and critical thinking skills in conjunction with professional judgement to provide competent clinical assessment for individuals presenting with undifferentiated, undiagnosed, and sometimes complex conditions to act as an enhanced nurse practitioner working independently and in conjunction with other health care professionals
To assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.
Clinical decision making and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice underpinned by theory and experience.
To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better
