Conduct in-depth assessments of referred families
Develop and implement collaborative family service plans
Provide home-based services including parent training and family counseling
Link families to mental health, school, and community-based supports
Maintain detailed client records and documentation
Coordinate case conferences and follow through to service termination
- Implement the plan through the provision of home-based services, including individual and family counseling, parent training, and linkage with schools and community agencies in collaboration with Family Preservation team.
Bachelor's Degree required, Masters, preferred in Social Work.
Experience with at-risk families, mental health, and service coordination.
Strong time management, communication, and relationship-building skills.
Ability to work flexible hours and participate in trainings.
Valid driver’s license, reliable transportation, and insurance required
Comfortable working with databases and Microsoft Office tools
