- FSS/FAW Specialist will conduct Family Assessment Worker (FAW) assessments or hospital engagement and referral collection while covering Family Support Specialist (FSS) home visits of families.
- Establish positive and trusting relationships with families; in consideration of families’ personal experiences and diverse cultural backgrounds.
- Enroll families in the Healthy Families program and meet with families in the hospital to engage families and gain referrals into the program.
- Engage in face-to-face weekly home visits for families needing coverage due to prior FSS unavailable for extended periods. Provide flexibility in schedule to allow for the movement of family visits.
- Compose accurate and complete in-depth documentation required for all families enrolled and met with for weekly home visits. This will consist of assessments, home visit notes, program intake forms, data tracking regarding the status of screening/assessment activity, within appropriate timelines, and other data forms as needed.
- Receive reflective supervision by the FAW manager and FSS manager to get the appropriate support for families enrolled and visited.
- Bilingual Spanish/English.
- High School Diploma or equivalent + 3 years of experience in social services OR associate's degree or higher in human services or a related area.
- Experience working with culturally and linguistically diverse populations.
- Attention to detail with the ability to perform assignments and job responsibilities efficiently, accurately, and on time.
- Valid US state driver’s license
- Reliable and registered vehicle with valid auto insurance.
- Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card.
