What to Expect (Job Responsibilities):
- Conduct initial and ongoing assessments for long-term services and support (LTSS) using person-centered approaches
- Establish goals and priorities with members through telephonic and in-person interactions
- Develop and continuously modify service plans in collaboration with care team members
- Assist members in navigating LTSS and related social and environmental needs
- Support members in maintaining Medicaid eligibility and accessing necessary services
What is Required (Qualifications):
- Bachelor's degree in a health and human services field
- Minimum of 2 years of related experience
- Proficient in Microsoft Office Word and Excel
- Exceptional verbal and written communication and interpersonal skills
- Valid state driver's license and proof of personal vehicle liability insurance
How to Stand Out (Preferred Qualifications):
- Experience with electronic case note documentation and multiple computer applications/systems
- Background in health promotion, coaching, and wellness
- Knowledge of community health, social service agencies, and additional community resources
