Job Skills / Requirements
Full-time position available requiring excellent communication skills both verbal and written, professional work ethic, and accurate record keeping abilities.
The Service Coordinator will be responsible for:
Receiving and responding to all referrals within 2 business days.
Completing data entry.
Conducting and completing intake.
Collecting needed documentation.
Arranging for and ensuring completion of necessary assessments and evaluations.
Experience working with infants, toddlers & their families or experience serving persons with disabilities preferred. Must have valid driver's license and dependable transportation.
This role will be required to travel to clients' homes to provide services.
Bachelor's degree required.
The schedule is Mon-Fri 8am-4:30pm with flexibility for evening hours as needed.
