Job Description

- Symbria Rehab/SeniorFITness Program Specialist Certification is required within six months of hire.
- Provide education sessions to participants and staff on benefits of program services.
- Actively market and promote program services to potential participants in order to increase program enrollment.
- Complete enrollment and medical screening process for each participant according to Best Practice Standards.
- Inform participants of Membership Contributions.
- Track and communicate changes in member’s health status with physician or healthcare provider.
- Schedule and conduct fitness assessments and record data into Fitness Outcome Measurement System (FOMS).
- Coordinate and administer individual exercise program consultations for members based on current ACSM/AHA guidelines and Best Practice Standards.
- Train and Certify seniors on proper and effective use of fitness equipment by providing a quality exercise prescription based on current ACSM and SeniorFITness guidelines.
- Update participant program goals on quarterly basis and provide ongoing progress/program outcomes to participants.
- Assist, encourage and motivate any and all seniors with their specific needs in the fitness program at any time. Help seniors identify and overcome barriers to adherence to the exercise program.
- Achieve/exceed company stated benchmarks for participant utilization and satisfaction.
- Track and report statistics related to all biometric testing/utilization on weekly basis.
- Report absences, substitutions, changes or issues immediately to the Program Director, who will then inform CDoFSS Supervisor of Senior Fitness.
- Report any equipment malfunction or maintenance issues to the CDoFSS Program Director and to KC Fitness Service
- Follow the Preventive Maintenance Schedule for the Keiser Machines at all times possible; about 15 minutes a day should be dedicated for maintenance.
- Attend all Symbria Rehab scheduled meetings and training workshops.
- Teach and personally adhere to the training and biomechanics principles taught by the Program Director.
- Assist in the development of fitness programs for seniors with chronic disease and disabilities.
- Follow emergency procedures and safety precautions provided by CDoFSS and Symbria Rehab.
- Maintain accurate records of attendance and unduplicated counts (Master List) of participants by each site.
- Submit all Monthly Site Reports and Timesheets by the due date.
- Provide appropriate program service instruction as trained and assigned.
- Provide group exercise class instruction and specialty programs as appropriate for site initiatives
- Assist in the training and supervision of student interns.
- Maintain punctuality by arriving at each site 5‐10 minutes early to allow time for proper record keeping procedures.
- Any and all duties as assigned.
- American College of Sports Medicine (ACSM) Health Fitness Instructor and National Strength and Conditioning Association (NSCA), American Council on Exercise (ACE), American Fitness and Aerobics Association (AFAA) Personal Trainer or Group Instructor certification required. Will consider someone wtih experience seeking certification.
- Bachelor’s degree in Exercise physiology or related science preferred.
- CPR and First Aid certification.
- Bilingual Spanish/English Required
- Experience working with older adults and desire to improve the overall health and well‐being of older adults.
- Working knowledge of variable resistance exercise machines, free weights and cardiovascular equipment.
- Strong communication and organization skills.
- Proficient use of computers and Microsoft Office: Word, Excel and Outlook.
- Excellent customer service and relationship building skills.

Symbria Rehab is seeking a BILINGUAL SPANISH/ENGLISH Per Diem Fitness Instructor/Personal Trainer seeking flexible schedule of 2-15 hours per week who has a passion working with older adults, and looking to make an impact of people's quality of life through fitness and wellness programs. Great opportunity in several locations in Chicago including Northeast, Northwest, Southeast, and Southwest
Must be willing to be available for travel depending on need. This is NOT a remote role.
To deliver SeniorFITness wellness programs and fitness services to participants at assigned client site(s) while representing Symbria Rehab Core Values in activities of daily work.
This position is a partnership between Symbria Rehab and the Chicago Dept of Family Support and Services (CDOFSS)
