- Perform thorough health assessments in patients' homes, including reviewing medical histories, conducting physical exams, and screening for chronic conditions.
- Educate patients on medication management, disease prevention, and overall health maintenance.
- Document relevant diagnoses and communicate findings to ensure effective care management.
- Collaborate with primary care providers to address any gaps in patient care.
- Enjoy a flexible schedule—no on-call, weekends, or holiday shifts required.
- Availability to complete at least 30 in-home assessments per month.
- Active Nurse Practitioner license in Maine.
- Board Certification from AANP or ANCC in Family, Adult-Gerontology, or related specialties.
- Minimum of 1 year of clinical experience, preferably with geriatric patients.
- Strong communication and interpersonal skills, with a dedication to delivering patient-centered care.
- Reliable transportation for local travel.
- Comfort with working in varied home environments.
- Competitive per-assessment pay ranging from $80 - $90, with potential for additional compensation per screening.
- $500 sign-on bonus after completing 30 assessments.
- Flexible, self-scheduled work hours.
- Mileage reimbursement for travel beyond 50 miles.
