Job summary
We are seeking a Frailty Nurse to join our well-established Assessment and Care Team working at a PCN level to support patients registered at our four practices.
The role will be based at our Network Hub at East Way Clinic but will also involve community visits to patients at their homes.
Main duties of the job
You will be supporting the housebound patients and residents in care homes as part of our Assessment and Care Team. This will include care planning, vaccinations, helping patients to live independently, managing long term conditions and reducing unnecessary admissions. About us
Central Bournemouth Primary Care Network is made up of four GP surgeries - Panton, James Fisher, St Albans and Moordown. We have approx 50,000 patients registered across our network.
This role is available at full time but we are able to offer part time including school friendly hours. The Assessment and Care Team is comprised of a Team Lead Nurse ANP, Frailty GP, Frailty Nurse ANP, a Trainee ANP, Associate Practitioner and a Care Co-Ordinator.
