- Undertake clinical management of the patients as appropriate including initiating investigations, reviewing results, and making referrals to secondary care or to other providers as appropriate
- Develop care and treatment plans in consultation with patients and in line with current practice disease management protocols.
- To undertake duties governed by the contract and Direct and Local Enhanced Services including:

Job summary
We are seeking a dedicated and enthusiastic GP who has completed their training (CCT) within the last 2 years to join our friendly and supportive team for 4-5 sessions per week. You will be based in our new, purpose-built health centre and will be well supported by an experienced clinical and administrative team. The multidisciplinary clinical team benefits from a diverse skill mix, comprising General Practitioners, Clinical Pharmacists, a Physician Associate, a First Contact Physiotherapist, Practice Nurses, Healthcare Assistants and a GP Assistant for clinical document management, together with administrative support.
We also work closely with local organisations to offer a range of activities from our community rooms on the ground floor that is ideal for promoting wider health and well being.
