      Job summary

      Come and work for a completely new and exciting integrated team aimed at driving and transforming the current healthcare delivery system by embedding the use of digital technology and remote patient monitoring systems to release pressure from acute hospital and improve patient outcome.

      The team is a part of an integrated system between BHRUT & NELFT, both known for their work in the field of diversity and inclusivity. This service will be the first of its kind bringing integration between the Acute and Community providers in the London boroughs of Barking & Dagenham, Havering, Redbridge and Waltham Forest.

      Main duties of the job

      Responsible for the overall safe and smooth delivery of the Frailty Virtual Ward at the operational level.

      To work with clinical colleagues within the MDT to identify patients who are elderly, frail or who have long term health needs that would be suitable for enrolment in the virtual ward.

      To support the transfer of patients on to the virtual ward

      To undertake virtual welfare checks for patients on the virtual ward

      To co-ordinate care plans, making sure actions are completed by health care professionals

      To coordinate, attend and provide administrative support for MDT meetings. To disseminate information from these meetings to others as necessary

      To coordinate visits or arrange appointments for patients on the caseload

      • Review data available within the service to report, monitor and trac performance and to satisfy reporting requirements within the designated areas of responsibility.

      • To be responsible for a high-quality customer service function in supporting the initial prioritisation of contacts, subsequent feedback and on-going liaison with referrers and relevant others.

      • Ensure accurate and up-to-date patient details are maintained on patient information systems in line with Trust Information Governance policy.

      • Maintain health records and patient files in line with Trust Health Records Policy.

      We provide care for the residents of three diverse London boroughs. Most of our 7,800 permanent staff live in Barking and Dagenham, Havering and Redbridge and the majority are from black, Asian and minority ethnic groups. We also provide healthcare services to people in south west Essex, and specialist neurosciences services to the whole of the county.

The Elective Surgical Hub at King George Hospital is one of eight to be accredited as part of a national scheme; the Care Quality Commission has raised the ratings forurgent and emergency careat Queen's and King George hospitals; anddata released by NHS Englandshowed that the Trust was the most improved in 2023 for reducing waits for emergency care.

      We are particularly proud of our regional Neurosciences Centre; Radiotherapy Centre; Hyper Acute Stroke Unit; and dedicated breast care service at King George Hospital. We're also part of the North East London Cancer Alliance.
