      Job summary

      We are looking for a GP to cover Maternity Leave for our DTA (Discharge to Assess) and CBU (Challenging Behaviour Unit) patients. These are patient's who are discharged from hospital but are not yet ready to return home or become permanent Nursing Home Residents. We cover a number of Nursing Homes in Urmston, Stretford and Altrincham.

      Main duties of the job

      We provide three ward rounds per week on a Monday. Wednesday and Friday assessing the DTA patients, issuing and making changes to medications, attending MDT meetings, making referrals etc.

      About us

      At the Delamere Medical Practice we have around 15 thousand patients. Our Clinical team includes 5 partners, 4 salaried GPs, 3 Practice Nurses, 1 Care Coordinator, 4 GPAs, 1 PA, 1 FCP and 1 Clinical Pharmacist.

      We have a Reception Team, an Admin Team, 2 Secretaries and a Medicines management team.
