      Main duties of the job

      • Conduct nurse-led clinics for long-term condition reviews (e.g. diabetes, COPD, hypertension)
      • Deliver immunisation and vaccination programmes for all age groups
      • Support health promotion and screening initiatives (e.g. smoking cessation, NHS Health Checks)
      • Perform venepuncture, ECGs and point-of-care testing
      • Assess and manage leg ulcers, applying and reviewing complex dressings
      • Lead and execute comprehensive infection control processes and audits
      • Maintain accurate clinical records and ensure adherence to practice protocols
      • Collaborate with GPs, HCAs and other team members to coordinate holistic patient care.
      • Arrange for and perform appropriate tests as indicated such as blood, swabs etc.
      • Make referrals as necessary to other health and social care professionals
      • In participation with the Practice Manager, ensure the maintenance of equipment and stock levels relating to patient care
      • Undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness and stocking of equipment used
      • Perform any investigatory procedure requested by the GPs such a spirometry., ECGs, 24 hour blood pressure monitoring etc.
      • Ensure accurate completion of necessary documentation associated with patient health care or registration, supplies, equipment calibration etc.
      • Attend and participate in practice meetings as required

      1. Role Purpose

      As Practice Nurse at Longcroft Clinic, you will deliver high-quality, patient-centred nursing care across a broad spectrum of clinical services. You will lead nurse-led clinics, manage long-term conditions, perform specialist procedures, and champion infection-control standards. Working closely with GPs, nursing colleagues and healthcare assistants, you will be instrumental in driving service improvements, mentoring junior staff, and ensuring our patients receive seamless, evidence-based care.

      2. Key Responsibilities

      • Chronic Disease Management
        • Plan and run nurse-led clinics for diabetes, COPD, hypertension, CKD and other long-term conditions
        • Review clinical parameters, interpret results, and adjust treatment plans in collaboration with GPs
      • Immunisations & Vaccinations
        • Deliver childhood and adult immunisation programmes (including flu, travel and catch-up vaccinations)
        • Maintain cold-chain documentation and safety protocols
      • Wound Care & Complex Dressings
        • Assess and manage acute and chronic wounds, including leg ulcers
        • Select and apply advanced, complex dressings; monitor healing and modify care plans
      • Infection Control & Audit
        • Design, implement and lead comprehensive infection-control audits and action plans
        • Maintain and validate sterilisation/decontamination equipment; ensure full CQC compliance
      • Clinical Procedures & Point-of-Care Testing
        • Perform phlebotomy, ECGs, spirometry and finger-prick tests (e.g. INR, HbA1c)

      Registered Nurse (NMC Part 1) with valid PIN

      Minimum 2 years experience in UK primary care

      Proven competency in chronic-disease management

      Wound-care certification (e.g. Wound Care Lead) (desirable)

      Skilled in infection control processes, audit and equipment validation

      Experience with EMIS Web clinical systems

      Strong communication, organisational and IT skills

      Leadership, mentoring or project-management experience
