Job summary
We are looking for an enthusiastic salaried GP with a view to future partnership - to join our friendly, innovative practice of just under 7500 patients located in Maswell Park Health Centre, situated on the edge of Hounslow and Richmond boroughs. The post is for 4-6 sessions a week, at a session rate starting at £11,500 per annum pro rata, days and times can be negotiated.
Our clinical team is made up of doctors, nurses, and HCAs, and are very well supported by our management, admin and reception team. As a practice we strive to provide forward thinking, compassionate care for our patients and their families. We are mindful of maintaining a healthy work balance and have adopted largely 15 minute appointments, a pharmacy service to manage our repeat prescribing and embedded a document workflow team to reduce admin burden on the clinical team. We are members of a proactive PCN and work closely with the pharmacists, first contact physiotherapists and social prescribers attached to our network. We are an approved training practice with undergraduate teaching with Imperial Medical School, supporting students in years 1 and 2 for placement. We are keen that the successful candidate has an interested in education, continuity of care and project work and is looking for a long term commitment.

Main duties of the job
Main Duties
Clinical responsibilities
1. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team
2. Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
3. Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate
4. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice
5. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
6. Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children
7. Support the delivery of advanced care plans
8. Recognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health records

Clinical Responsibilities:
In accordance with the Practice timetable, as agreed, the post-holder will make him/herself available to undertake a variety of duties including surgery consultations, online consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions
Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
