      Job summary

      We are looking for a clinical Health Care Assistant with training and experience in phlebotomy and injections eg B12, to join our vibrant practice.

      Health Care Assistants in our practice are involved in more aspects of patient care, such as:

      • take blood samples
      • carry out diabetic foot checks
      • give injections (flu / covid/ B12 etc)
      • test urine samples
      • prepare samples for the pathology lab
      • take Blood Pressures and escalate to GP's if out of range
      • carry out NHS Health Checks
      • maintain room stocks
      • perform CQC checks and monitoring

      You will work with and be supported by a team of nurses, nurse associates, senior HCA's and HCA in our busy and fast paced practice making sure that the patient experience is comfortable and stress free.

      Main duties of the job

      To provide Health Care Assistant (HCA) services to the practice patients.

      Supported by team members our Health Care Assistants (HCA) deliver care within the boundaries of their role and in accordance with specific practice guidelines and protocols.

      Responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making within the parameters of their qualifications and role requirements.

      They will focus upon supporting patients to be healthy by assessing their needs, providing HCA care, health education advice, screening activities, implementing care at the surgery, community or other settings, maintaining clinically accurate records and running clinics.

      They will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures, and providing HCA guidance as required.

      About us

      The Park Surgery is a forward thinking doctors surgery in Driffield set on the edge of the Yorkshire Wolds,looking after approx 15000 patients, with a branch surgery at Nafferton. We have our own dispensary and prescribe to about half our patients from here.

      The practice has 3 GP Partners and a diverse mix of salaried GP's, ANP's, Nurses, HCAs, Pharmacists, pharmacy technicians who are all supported by a Management Team and back office staff such as admin, care coordinators, reception, finance and the dispensary team.

      We are also a training practice and work with HYMS developing the GP's of tomorrow.

      Clinical Practice

      The post holder will provide safe, evidence-based, individualised patient care following policies and procedures, to include:

      Phlebotomy

      o Taking blood samples

      o Processing for pathology using ICE system and following clinician requests

      o Checking samples and labels are correct

      o Using centrifuge to spin and process bloods

      Samples

      o Following standard operating procedures for urinalysis

      o Preparing samples for pathology testing

      o Checking for accuracy with labels

      Vaccinations (given under a PSD) (min level of HCA experience required)

      o Participate in seasonal vaccinations such as Flu and Covid

      o Administer injections such as B12, pneumonia, shingles

      Long Term Condition Initial Assessment Chronic Diseases.

      o agreed guidelines for patients with chronic diseases eg CHD, Diabetes and Hypertension.

      o Diabetic foot checks

      o Health and lifestyle questioning

      o Take blood pressure readings and follow protocols for escalation

      o Anticoagulation monitoring taking samples for INR dosing and contacting patients with amended dose instructions

      o Participating in learning disability and dementia review work

      o Participating in the Practices Quality Outcome Framework success

      o Carry out routine ECG tests following protocols

      Minor Surgery

      o Assist GP in providing minor-surgery sessions, coil fits and Implanon fittings

      Emergency Care

      o Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

      Wound Management

      o Removal of stiches and clips

      o Assessing wounds, taking swabs, wound cleansing and dressing changes

      o Following dressing formulary

      o Escalating to clinicians and notifying when referrals to TVN is needed

      Mental health, adult and child safeguarding

      o Recognise patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health. Raise relevant information with the duty GP.

      o Recognise adult and child safeguarding situations and act appropriately.

      IT and Record keeping

      Use of Practices clinical IT system (Emis)

      Ensure accurate recording of consultations by completing Templates (Ardens) and notes within the patient record.

      Completing Tasks set in the clinical records

      Ensuring confidentiality and Caldicott principles are maintained

      Communicating with team members and patients using the AccuRx digital communication platform.

      Governance and Infection Control

      Support the infection control lead in the practice.

      Ensure that rooms are kept to a high standard of clinical hygiene and reporting issues to the cleaners where it is their responsibility.

      Undertake regular routine disinfecting of clinical areas in accordance with infection prevention control.

      Reporting damaged / broken items and removing them from use

      Checking stock is in date and fit for purpose

      Maintaining stock levels, stock rotation and ordering supplies

      Re-stocking rooms in the practice

      Maintain clinical equipment and ensure servicing and calibration are carried out annually

      Fridge monitoring and ensure that vaccines are stored correctly for temperature optimization

      Understand, follow and maintain Cold Chain procedure for receipt, storage and escalation of fridge excursion.
