      The Ohio Department of Health supports a hybrid teleworking model. This position may work a hybrid schedule at the discretion of the manager or supervisor, which will require occasional attendance in the office for training, meetings and other operational needs. Employees must reside within a reasonable distance to their assigned headquarters for compliance with workplace reporting requirements.

      What You'll Do:

      • Conducts interviews and investigates and analyzes complaints/incidents of unsatisfactory services (e.g., resident abuse and neglect, misappropriation of funds, violation of rights). Using core knowledge as a registered nurse, dietitian, social worker, or registered sanitarian determines immediate medical assessment at the time the complaint/incident is received to determine the severity and urgency of the situation.
      • Collects and classifies information to obtain clear, accurate and concise personal demographics from the complainant (e.g., patient name, facility, allegation, dates and time of the occurrence, staff involved). Reviews, assesses, analyzes initial allegation and establishes fact pattern (e.g., utilizes investigative techniques to determine and ensure compliance with state and federal laws addressing patient rights, neglect and abuse, and care and services of the patient, other patient related services). Maintains all confidentiality requirements (e.g., HIPAA) in dealing with all complaint/incident information.
      • Researches special problems and completes reports as directed: obtains information by telephone to clarify allegations as necessary; performs other duties associated with Medicare/Medicaid certification and licensing (e.g., organizes certification packages and files; writes summary of case status; researches regulations and regulatory issues; notifies attending physicians and Board of Examiners of Nursing Home Administrators when substandard quality of care has been identified in long term facility; recommends disapproval or revocation of facility dietician aide training/testing).
      • Provides input to supervisor regarding procedures involving enforcement actions; prepares and delivers speeches to consumer and provider organizations; attends surveys as observer in order to maintain familiarity with survey when assigned.
      • Operates a personal computer (PC) using form letters and the federal complaint/incident tracking system. Utilizes office equipment (e.g., computer e-mail, fax, copier, scanner) to distribute documentation of the complaint/incident as necessary. Prioritizes and forwards the completed complaint/incident intake information to the appropriate location for continued processing (e.g., supervisor for review, scheduling of an on-site investigation, Abuse, Neglect, and Misappropriation section, Centers for Medicare and Medicaid Services [CMS], Office of the Attorney General).
      • Responds to inquiries from the public, complainant and/or facility representatives as to the status of complaints/incidents in progress or forwards such contacts to the appropriate investigating office. Provides input to supervisor regarding procedures involving enforcement actions; prepares and delivers speeches to consumer and provider organizations; attends surveys as observer in order to maintain familiarity with survey when assigned; orients other division enforcement and survey staff regarding enforcement actions.
      • May be assigned on a temporary basis to perform field surveyor assignments. As assigned, serves as a member of a disaster recovery/business resumption team, incident response team, or similar public health response team which may include the conduct of operations on a 24/7 basis at remote locations.

      Certification, licensure, or registration commensurate with assigned discipline (i.e., registered nurse, or licensed dietitian, or licensed social worker, or registered sanitarian)

      12 mos. exp. as Health Care Facilities Field Surveyor – Entry, 61581

      Successful completion of federal Surveyor Minimum Qualifications Test (SMQT)

      • Bachelors or advanced degree in Nursing, Human Nutrition/Dietetics or Social Work
      • Experience conducting health care facility (nursing home, ICF/IID, residential care facility), life safety code, or clinical laboratory (CLIA) surveys
      • Has passed the Surveyor Minimum Qualifications Test (SMQT)
      • Experience working with healthcare complaints e.g., gathering information, responding to customers
      • Experience working with computer software (e.g., Word, Excel, ASPEN)
      • Experience in communications with various levels of technical and non-technical
      • Experience and/or education in technical or report writing
      • Experience working in a nursing home or ICF/MR as a licensed health care professional
      • Experience working in critical care, hospital, urgent care or emergency department as a licensed health care professional
      • Experience in providing training and orientation to new employees or cross training to established employees

      Unless required by legislation, the selected candidate will begin at Pay Grade 13, Step 1 of the union (1199) Pay Range Schedule ($31.86 per hour) with an opportunity for pay increase after six months ($33.40) of satisfactory performance and then a yearly raise thereafter.
