Position Description:

• Provide case management and care coordination, health education, interdisciplinary collaboration, coordination, consultation, and administrative duties
• Work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team
• Act as a liaison between Talbert House and the VA or medical clinic, and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans
• Provide timely, appropriate, Veteran centered care equitably
• Work collaboratively with the team and the Veteran to identify and address system challenges for enhanced care
• Participates in the Development and/or implementation of individualized client service plans and ensure compliance with applicable standards and agency policies
• Maintain progress, maintains comprehensive documentation, and provides information to treatment team members
• Participate in treatment team and assist in making recommendations for client status changes
• Focuses on community services, outreach, and referrals needed for the Veteran
• May require transportation of clients in vehicle
• Adhere to professional standards as outlined by protocols, rules and regulations (e.g. critical incident reporting, HIPPA)
• Other duties as assigned

Job Requirements:

• Masters level social worker or equivalent education and experience is preferred
• 2 years of experience in a healthcare or social services area of practice
• Valid driver’s license and current auto insurance

Essential Functions/Physical Demands:

• Positional: Driving in accordance with job duties assigned. Occasional standing and walking. Occasional to frequent sitting.
• Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching and handling.
• Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
