- Provide case management and care coordination, health education, interdisciplinary collaboration, coordination, consultation, and administrative duties
- Work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team
- Act as a liaison between Talbert House and the VA or medical clinic, and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans
- Provide timely, appropriate, Veteran centered care equitably
- Work collaboratively with the team and the Veteran to identify and address system challenges for enhanced care
- Participates in the Development and/or implementation of individualized client service plans and ensure compliance with applicable standards and agency policies
- Maintain progress, maintains comprehensive documentation, and provides information to treatment team members
- Participate in treatment team and assist in making recommendations for client status changes
- Focuses on community services, outreach, and referrals needed for the Veteran
- May require transportation of clients in vehicle
- Adhere to professional standards as outlined by protocols, rules and regulations (e.g. critical incident reporting, HIPPA)
- Other duties as assigned
- Ability to evaluate the effectiveness of the resources and referrals provided and make appropriate modifications to ensure high-quality care and interventions
- Ability to function independently, exercising initiative and judgment in day-today activities
- Excellent organizational and time management skills
- Effective verbal and written communication skills
- Ability to relate to and interact with a wide variety of people
- Advanced level problem solving skills and ability to multi-task while prioritizing
- Ability to utilize technology including electronic service documentation system
- Masters level social worker or equivalent education and experience is preferred
- 2 years of experience in a healthcare or social services area of practice
- Valid driver’s license and current auto insurance
- Positional: Driving in accordance with job duties assigned. Occasional standing and walking. Occasional to frequent sitting.
- Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching and handling.
- Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
Position Summary:
The SSVF Health Care Navigator will provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. They will serve as the subject matter expert on community resources related to the needs of the VeteransPosition Description:
