Job Description: A Health Care Occupational and Case Manager is responsible for coordinating and managing the care of patients with complex medical conditions or disabilities. They work closely with patients, healthcare providers, and insurance companies to ensure that patients receive the appropriate care and support to improve their health outcomes. Responsibilities may include:
Assessing patients' medical, psychological, and social needs.
Developing individualized care plans in collaboration with healthcare providers and patients.
Coordinating healthcare services, including medical treatments, therapies, and consultations.
Monitoring patients' progress and adjusting care plans as needed.
Providing education and support to patients and their families.
Advocating for patients' needs and rights within the healthcare system.
Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
Evaluating the effectiveness of care plans and making recommendations for improvement.
Documenting patient assessments, care plans, and interactions.
Keeping abreast of healthcare regulations, policies, and best practices related to case management.
Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management. They should have strong communication, organizational, and problem-solving skills. A compassionate and empathetic approach to patient care is essential. Depending on the specific role, certification as a Case Manager (CCM) or in a related field may be required or preferred.
