      Job summary

      An exciting opportunity has become available for a suitably experienced Health Care Support Worker to work within the Pembrokeshire Older Adult Community Mental Health Team.

      Full time and part time hours will be considered.

      The successful candidate will display a willingness to undertake further training and personal development and to engage in supervision.

      The post holder will be required to support colleagues in delivering a high standard of person centred care in accordance with Care and Treatment Plans. Excellent interpersonal and communication skills are essential as is the ability to work autonomously as well as under supervision.

      Main duties of the job

      You will need to be flexible to the needs of the service and have experience of working with Older Adults with enduring Mental Health problems and/or Cognitive impairment including Behavioural and Psychological Symptoms of Dementia (BPSD), experience of working within a community setting would be advantageous.

      You will be expected to work with a caseload of patients under the supervision of identified Care Co-ordinators.

      Main duties will cover:

      Promoting health and well-being.

      Encouraging patients to do as much as they can in every day tasks .

      Building therapeutic relationships with patients and their families.

      Assist in patient recovery through meaningful activities and engagement.

      Assist in patient recovery through escorting them on S17 leave to the community.

      Carrying out observations including level 3 and 4.

      Monitoring their safety and well-being.

      Provide concise and relevant information of patients to the care co-ordinator and relevant professionals, documenting electronically on carepartner.

      Transporting and escorting patients for community activities.

      48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.

      Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
