      What to Expect (Job Responsibilities):
      - Serve as a Patient Navigator, coordinating with medical and mental health providers to manage patient referrals and intake
      - Provide indirect clinical support by connecting patients and families to case management services, including referrals and assistance
      - Follow up with high-risk patients to support safety planning and crisis management
      - Maintain accurate records and track the status of referrals
      - Develop outreach materials to educate patients and families about available specialty services

      What is Required (Qualifications):
      - Bachelor's degree in social work or related field plus two (2) years of mental health experience, or High School diploma/GED plus six (6) years of mental health experience
      - Mental health paraprofessional certification within 90 days of hire
      - Proven experience in managing patient referrals and intake processes
      - Strong communication skills for coordinating with various healthcare providers
      - Ability to connect patients and families with necessary resources and support services

      How to Stand Out (Preferred Qualifications):
      - Experience in outpatient behavioral health programs
      - Experience working with preschoolers, children, and teens in mental health or social service settings
