What to Expect (Job Responsibilities):
- Process all patient referrals according to established protocols
- Coordinate with various healthcare providers and maintain accurate records of referrals
- Educate patients and families about available specialty services
- Schedule appointments and coordinate with support staff
- Connect patients and families to necessary resources, including crisis intervention services

What is Required (Qualifications):
- Bachelor's degree in social work or a related field plus two (2) years of mental health experience, or
- High School diploma/GED plus six (6) years of mental health experience
- Mental health paraprofessional certification within 90 days of hire

How to Stand Out (Preferred Qualifications):
- Experience in outpatient behavioral health programs
- Experience working with preschoolers, children, or teens in mental health or social service settings
