Job summary
An exciting opportunity has arisen to join the Health Facilitation and LiaisonTeam, providing support to people with a learning disability when in accessing primary and secondary care.
This post comes during a period of Service Transformation in response to the changing needs of people with a learning disability and the national agenda for improvement. If successful, you will play a vital part within this exciting period of change, by contributing to the design of a new service model providing flexible & responsive services.
The post holder will;
- Provide a health facilitation and/or liaison service to people with a learning disability who need to access primary or secondary care.
- Work in collaboration with acute and secondary care providers to ensure reasonable adjustments are made in delivery of care to people with a learning disability and their carers.
- Be expected to deliver training to acute and primary care services staff.
- Directly assess patients referred to the service from acute and secondary care and provide advice and guidance on making reasonable adjustments to care for the patient.
Various full and part time hours are available.
Main duties of the job
You will be responsible for ensuring people with learning disability receive an equitable healthcare experience
You will also be responsible for providing training on the needs of people with learning disability to our acute and primary care services staff.
You will be expected to provide direct support to individuals when accessing health services.
You will be responsible for trying to reduce health inequalities and reduce avoidable deaths for people with a learning disability.
You will be required to liaise with a vast number of professionals to ensure the individual receives a high standard of care.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
