      Job summary

      We are seeking an enthusiastic Cardiac Nurse to join our busy community Heart Failure team to help us provide exceptional patient-centred care. You will work within our specialist service playing a crucial role in patient assessments and supporting the delivery of individualised management plans at home or in clinic settings.

      As part of a skill mixed team you will be committed to the keeping patients out of hospital and in their preferred place of care. Youll work closely with a skill mixed specialist team to provide holistic care, education, and support to patients and their families. Your expertise will help improve patient outcomes, enhance quality of life and the patient experience.

      Main duties of the job

      To work in partnership with the Heart Failure Specialist Nurses in providing specialised evidence based care in accordance with the clinical management plan as directed from the HFSNs assessment. The aim is to provide care and interventions until the patient reaches stability.

      To work independently managing a caseload of Amber patients and prioritising clinical need by using specialist assessment skills and evaluations.

      To assist in the promotion of early identification and interventions to reduce episodes of decompensation by working closely with the HFSN and responding proactively to changes in condition and symptoms of fluid retention.
