  Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities.   Position Summary:

  The Health Information clerk is responsible for timely and accurate filing and retrieval of medical charts, collating and retrieving medical information, obtaining accurate and appropriate releases for medical records, providing back-up for Switchboard Operator, delivery of medical records to clinical areas, assisting with visiting providers and patients. Health Information clerk will also serve as phone technician for clinic providers where responsibilities include taking messages and forwarding electronically through EMR to appropriate staff.

  Essential Functions:

  • Recognizes and maintains patient confidentiality.
  • Accurately files charts in accordance with prescribed methodology.
  • Files medical information in patient chart in appropriate place.
  • Distribution of charts and other medical records upon request of clinical staff and other clinic staff in a timely and efficient manner.
  • Accurately obtains release for information and compiles records accordingly.
  • Uses appropriate communication skills, both verbal and written, in communicating with internal and external customers

  The responsibilities listed above are not all-inclusive; other activities may be required in support of the hospital’s goals and objectives.

  Position Requirements:

  • High School Graduate or equivalent
  • Knowledge of office practices and equipment-computer, fax, printer, scanner, switchboard

  Pay Range:

  Pay ranges from $15.00/hour to $20.603/hour (rate of pay is based on applicable years of experience)

  Hours: Monday- Friday 8a-430pm
