Job Summary:

The HIM Clerk is responsible for processing birth registrations and handling medical record release requests. This role ensures the accuracy and confidentiality of patient information while following hospital policies and HIPAA guidelines. Also assist with patient walk in request as well as answering the phone.

Key Responsibilities:

· Collect and enter birth information into the state’s electronic birth registration system.

· Work with parents to complete and submit birth certificate forms.

· Process and track medical record release requests from patients and authorized third parties.

· Ensure proper authorizations are received before releasing any patient information.

· Maintain confidentiality and accuracy of all documents and records.

Qualifications:

· High school diploma or equivalent required.

· Prior experience in healthcare or clerical setting preferred.

· Basic knowledge of HIPAA and patient privacy laws.

· Strong attention to detail and organizational skills.
