      Responsible for ensuring all medical records have been received in the department and that they are properly identified, prepares documents for scanning and ensures all documents have been scanned into the medical record with accuracy and according to established procedures.

      QUALIFICATIONS

      Education

      Required: High school diploma or equivalent

      Preferred: Medical terminology course completion


      Experience

      Required: Customer service experience

      Preferred:

      Three to six months related experience and/or training or equivalent combination of education and experience.

      Previous healthcare experience or office setting with document scanning equipment experience.


      Knowledge/Skills/Abilities
      Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
      Familiarity with medical terminology.
      Knowledge of basic clerical duties and skills such as typing, filing, faxing, photocopying, and answering phones.
      Ability to use Microsoft Outlook, Excel and Word and have basic computer operational knowledge.
      Highly organized and detailed oriented.



      JOB RESPONSIBILITIES
      Essential

      Utilizes the hospital information system Master Patient Index (MPI) to confirm patient data assuring accuracy of patient data, including accuracy of patient name, medical record number, name and visit dates and make corrections (both electronic and paper) as deemed necessary and according to procedure.

      Travels to multiple facility locations to retrieve and secure paper medical records.

      Accounts for and tracks receipt of inpatient and outpatient discharged charts.

      Prepares medical record and loose sheet documentation for scanning.

      Converts paper documents into electronic images using a scanner.

      Maintains the accuracy and integrity of the patient’s medical record by performing quantity and quality checks on records prior to and after scanning to ensure the document is legible and scanned to the correct location in the medical record.

      Validates all documents have been received from the nursing units and that the documents have been scanned into the system.

      Purges/pulls inactive medical records according to retention policy, shifts records as necessary to allow for an efficient and accurate filing system.

      Keeps equipment secure and safe at all times. Performs routine maintenance to ensure equipment functions properly.

      Locates, retrieves, prints and/or faxes/e-mails medical records for continuity of care purposes.

      Attends and participates in meetings.

      Maintains confidentiality and security of patient healthcare information.

      Answers telephones promptly and courteously. Handles inquires appropriately.

      Maintains and monitors designated fax queue. Processes in documents in the fax queue in a timely and efficient manner.

      Monitors, tracks and verifies that all newborn screenings are documented, processed and findings are received back.

      Monitors the New Born Screening Online Specimen Tracking (NBS-OST) system daily.

      Navigates Electronic Health Records software efficiently.



      Additional

      Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.

      Performs other duties as assigned.
