Job Description

- Provide housing case management services, support, and guidance for clients experiencing housing insecurity to establish stable housing and ensure medical compliance and treatment adherence.
- Coordinate delivery of multiple housing programs, including invoice and budget tracking and submission of programmatic reports
- Assess individual and regional service needs and implement a comprehensive plan to improve health outcomes and reduce barriers to medical visit and treatment compliance in conjunction with the Ryan White case management team.
- Collaborate with community-based organizations, as well as state, regional, and local government agencies in planning, implementation, and evaluation of comprehensive services for persons living with HIV/AIDS.
- Coordinate, facilitate, participate, and/or report on regional HIV meetings, including planning committees and advisory boards.
- Requires cultural competence in working with a diverse population, including, but not limited to individuals of all races, ethnicity, age, gender, gender identity, sexual orientation, and background.
- Requires the ability to establish and maintain respectful working relationships with program funders, medical providers, subcontracted agencies, and other community-based organizations.
- Must be able to develop and implement program policy and defend decisions tactfully.
- Requires working knowledge of Microsoft Word, Excel, as well as the ability to quickly adapt to the program’s client level database.
- The ability to sit, flex, stand, reach, and bend.
- The physical strength to lift, carry, and transport materials/equipment are equivalent to thirty (30) pounds.
- Visual acuity, auditory ability, and clear written and verbal communication ability.
- Manual dexterity and the ability for data input into various computer programs.
- The ability to maintain professional behavior at all times.
- Must reside within St. Clair County, IL or be willing to relocate to St. Clair County within six (6) months from hire date.
- A valid Illinois Driver’s License.
- A dependable automobile for use in position if needed.
- Auto ins u r a n c e in ef f ec t with at least the m i ni m um l i m i ts of l i a bi l i t y as outlined and required by the State of Illinois.
- The ability to work necessary hours to meet the position requirements including evenings and/or weekends as assigned.
- May require provision of services at off-site locations.
- Adherence to all local, state, and federal regulations pertaining to OSHA guidelines.
- Health Department employees:
- Must be current on CDC Healthcare Personnel Vaccine Recommendations, which include Hepatitis B, MMR, Td/Tdap, Varicella (chickenpox) and Meningococcal if over 65. Influenza and COVID vaccines are recommended and encouraged, but not required at this time and maybe subject to change.
- Are required to have a TB test upon hire and at separation of employment.
- All employees must adhere to the St. Clair County Personnel Code.
- Position contingent upon continued grant funding.

POSITION SUMMARY:
With minimal supervision, carries out assigned responsibilities of the HIV Program. Duties include policy development, implementation, and monitoring, direct service delivery as well as oversight of regional service delivery, invoice and budget tracking, submission of programmatic reports, collaboration with regional stakeholders, and involvement in regional advisory boards.
SUPERVISION EXERCISED:
This position provides no direct supervision of Health Department staff.
SUPERVISION RECEIVED:
The position is mostly self-directed with general supervision by the HIV Program Manager.

JOB REQUIREMENTS AND QUALIFICATIONS:
Requires a Master’s Degree from an accredited university in behavioral science, social science or a health-related field with one (1) year of full-time experience in a health-related field. Experience in HIV care services is preferred,

Requires a Bachelor’s Degree from an accredited university in behavioral science, social science, or a health-related field with three (3) years of experience in a health-related field. Experience in HIV care services is preferred.
