      The Position :u00A0 Under the supervision of home care RN, the Licensed Vocational Nurse (LVN) provides nursing services to participants in the home setting. The home care LVN follows the plan of care developed in the IDT and provides care within the LVN scope of practice as ordered by the participantu2019s provider; communicates effectively in the medical record and with all members of the home care team and other program staff to ensure that the participants are receiving care that is appropriate.


      Pay rate range for the HomeCare LVN at CEI is $28.14u00A0 - $ 42.19 per hr.u00A0 u00A0Pay rate range based on market for the HomeCare LVN position as well as experience, skills, abilities and work history.

      GENERAL DUTIES and RESPONSIBILITIES:
      u00A0
      Perform physical evaluation, including vital signs and blood glucose monitoring
      Chart observations of participantu2019s condition during every visit and in patient health record within required timeframes. Report changes in condition to RN supervisor
      Complete medication reconciliation
      Promptly notify providers of changes in participantu2019s condition including any wounds, physical or behavioral changes
      Administer medication, screening tests, and immunizations as prescribed
      Perform basic wound care as prescribed
      Coordinate and communicate as needed with RN, occupational therapists, physical therapists, physicians, social workers, dieticians and others on the participantu2019s care team
      Report to RN supervisor when objective findings indicate that DME, home care assistance, nutritional services would improve participants quality of life and ability to live in the community
      Listen to the participant and communicate participantu2019s questions, concerns and requests to the RN supervisor promptly
      Educate participants and their families on home care strategies and procedures
      Participate in department and agency meetings as assigned
      Complete all required paperwork as assigned within agency time frames
      Participate in all agenciesu2019 quality assurance performance improvement program (QAPI) as assigned
      Maintain participant confidentiality per HIPPA/agency guidelines
      Follow all CEI Policies and Procedures
      Comply with all agency training requirements
      Maintain safe working environment by following CEIu2019s safety P&Pu2019s
      Maintain a courteous, helpful and professional attitude on the job; display a willingness and ability to be responsive to all customer groups
      Perform other duties as assigned in a positive and helpful manner

      QUALIFICATIONS:

      Must be available weekends and evenings to work on call
      Certification in Blood Withdrawal and Intravenous Insertion by a CA Board of Vocational Nursing Accredited agency or school Required.
      Graduate of a school of professional nursing
      Have a current California Vocational Nurse License
      A minimum of one (1) year acute experience, and experience working with the frail or elderly population
      A minimum of 1 year of work experience as a home care LVN. Two or more years of relevant work experience may be substituted for this requirement
      A valid California Driveru2019s license and access to a vehicle
      Exercises initiative and flexibility
      Maintains eligibility for licensure by obtaining required continuing education units
      Well organized
      Ability to effectively problem-solve complex issues
      Strong verbal and written communication skills
      Ability to work as a team player in a multi-cultural, multi-disciplinary setting
      Interest, enthusiasm and experience working with frail, cognitively impaired older adults
      Ability to conscientiously follow directions with limited supervision
      Strong interpersonal skills and service orientation
      Ability to maintain accurate records
      Current CPR certification and First Aid training
