- Maintains with daily contact with case managers/discharge planners to offer home care assistance and assure clients referred for services are followed up on in efficient and timely manner.
- Coordinates care for clients going home from hospitals, rehab etc., who have chosen the agency's services.
- Documents initial assessment for home care needs and potential plan for services in compliance with the agency policy.
- Arrange service in accordance with the established admission criteria, client care needs, and client preference
- Obtain referral, orders for initiation of home care services in accordance of agency policy/procedure and third-party regulations.
- Must have minimum of two (2) years' experience in home care setting, customer service, sales/marketing role or relevant experience.
- Must have current Driver's license and reliable transportation to and from work site
- Well-developed oral and written communication skills.

You will be responsible in bringing new client/referrals, establishing rapport with hospitals case manager, social worker, physicians, hospice, clinic and rehab etc,.
