      Job Summary

      • Reporting to the Rehabilitation Team Lead, the Physical Therapist is responsible for comprehensive patient management including evaluation, diagnosis, and intervention, and patient/family education for development and implementation of individualized care planning in the home setting. Opportunities include working directly in a multidisciplinary fashion, including specialty care groups such as Parkinson’s Disease, Covid Recovery and Post-Acute Stroke care. The ideal candidate demonstrates strong clinical skills in Physical Therapy, with opportunity for clinical growth through education, in-services, and care management groups. This Home Care environment offers family-friendly, flexible hours.

      Minimum Qualifications

      • Education
        • Bachelors Degree or higher from an accredited physical therapy program.
      • Experience
        • Prior Home Care experience is preferred, or at least one year previous experience in an acute and/or rehab setting.
      • Licensure and/or Certification
        • MA PT license and BLS required within 30 days from hire.
