Job Description

- Provide health - related services as outlined in the plan of care , authorized by a physician , and established by a registered nurse or licensed therapist , under the direction of the Aide Matrix established by the NYS Department of Health.
- Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
- Immediately report changes in client’s condition to supervising professional.
- Ensure the safety of the client , clie nt’s family and/or the employee.
- Maintain HIP A A.
- Complete and submit necessary patient documentation including charting tasks and observations on android device.
- Attend ongoing in-service programs in a timely manner , to maintain certification.
- All other duties as assigned.
- High School diploma or GED preferred .
- Completion of an approved program of PCA or HHA certification as designated by NYS Department of Health.
- Valid Driver's License and reliable transportation.
- Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program . )
- Must have worked in a certified HHA or PCA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry) .
- Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
