      SALMON Home Care is seeking dedicated and compassionate Registered Nurse to join our team on a per diem basis

      The Home Health RN provides nursing care to patients in their home and coordinates the patient plan of care with other inter-disciplinary team members.

      Registered Nurse responsibilities:
      • Provides direct care and case management of assigned patients, including initial assessment and development of plan of care.
      • Evaluates patients' status and goals and revises the plan of care as needed and communicates with all appropriate team members.
      • Ensures patients meet home health eligibility and medical necessity guidelines.
      • Perform assigned duties, including medication administration, wound care, IV therapy, treatments, and procedures.
      • Provides assigned duties, including teaching and education to patient and/or caregiver on disease process, medications, diet, wound care and other treatments as ordered by the physician.
      • Completes and submits accurate, timely clinical documentation pertaining to patient interventions and care provided.
      Qualifications:
      • Current license as a Registered Nurse in Massachusetts
      • Minimum of two years' experience in delivering nursing care
      • Minimum of one year of Home Health experience preferred
