      Position Summary

      Under the direction of the Hospice Director or designee and on order of the physician is accountable for direct and indirect quality care for patients and families of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Accepts professional development as an integral part of nursing practice. Manages a caseload of patients living within the service area of hospice. Coordinates patient care activities with other members of the health care team in a collegial relationship to promote patient comfort and assist in the achievement of a comfortable and peaceful death. Upholds the standards of confidentiality, attendance and punctuality.


      Position Specific Responsibilities

      • Manages a nursing caseload as assigned by supervisor that includes patients requiring a broad spectrum of care, who reside in a variety of environments, and in a wide geographical area, by practicing good organizational and time management skills.
      • Using the nursing assessment and process, makes an assessment of the patient and family's needs to develop the nursing care plan.
      • Initiates the admission and plan of care.
      • Confers with physicians and other hospice team members to develop the interdisciplinary Hospice plan of care.
      • Evaluates on an ongoing basis the patient and family’s psychosocial, spiritual, and physical needs, including the need for respite care.
      • Initiates necessary referrals in a timely manner.
      • Teaches, supervises, and counsels the patient and caregivers regarding patient care in accordance with the patient and family's wishes including cultural and religious practices, emotional barriers, desire and motivation to learn, physical and/or cognitive limitations, communication styles, and language barriers.
      • Maintains patient clinical, progress, and itinerary records to meet JCAHO and Federal regulations.
      • Directs the care given by the Home Health Aides, respite aides and volunteers by providing verbal and written instruction.
      • Under the direction of the hospice case manager, updates the interdisciplinary plan of care every 14 days or as patient condition requires.
      • Informs the physician, members of the hospice team and other parties as indicated of changes in the patient’s condition and needs.
      • Rotates through weekend, holiday, and 24-hour call as assigned by supervisor and according to PAMC contract.
      • Performs procedures and treatments according to accepted organizational policies and standards.
      • Participates in the Continuous Quality Improvement process and other quality management activities as defined by the organization.
      • Participates as preceptor in the orientation of new staff and/or students as assigned by supervisor.
      • Demonstrates advancement of professional nursing practice through community, professional organizations, or health system committee activities.
      • Meets or exceeds department productivity standard as defined by patients per caseload.
      • Maintains and demonstrates an adequate knowledge base for EOL care: pain and symptom management, bereavement, ethics, hospice philosophy, community resources, and reimbursement.


      Education and/or Experience

      • Required - Graduate of an accredited school of nursing
      • Preferred - Experience in Oncology, Hospice, Long Term Care or Homecare


      Skills

      • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
      • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
      • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
      • Other Qualifications: Knowledge of infusion therapy desirable.
      • Other Skills and Abilities: Must be able prioritize work assignment and must be able to meet department productivity standards.
      • Other Skills and Abilities: Must be self directed and have ability to work with minimal supervision.
      • Other Qualifications: Must have reliable transportation and automobile insurance according to AK State Motor Vehicle laws.
      • Other Skills and Abilities: Must have Pharmacology, Phlebotomy, and IV skills.
      • Other Qualifications: Must be adept at physical assessments and have knowledge of EOL care.

      Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
