      Responsibilities

      What You’ll Do:

      • Build and nurture relationships with referral sources including physicians, hospitals, senior living communities, and long-term care facilities
      • Educate referral sources on Advantage’s hospice services and patient care philosophy
      • Serve as a liaison between our clinical teams and community partners to ensure patient needs are being met
      • Represent Advantage in the community through in-services, presentations, and outreach events
      • Monitor referral quality and satisfaction, proactively resolving any concerns
      • Track and document contacts, referrals, and relationship-building efforts
      • Collaborate with leadership on new program development to meet evolving community needs

      Qualifications

      About You:

      • Experience in hospice, home health, healthcare sales, or community outreach (preferred but not required)
      • Strong communication, presentation, and relationship-building skills
      • Highly organized and self-motivated with a team-oriented approach
      • Ability to manage multiple relationships and priorities in a fast-paced environment
      • Familiarity with CRM tools, Microsoft Office, and professional reporting
