JOB SUMMARY: The Hospice Liaison’s (HL) primary responsibility is to advocate for the Medicare Hospice benefit and educate on services available from Bristol to members of the healthcare and patient community. The HL accomplishes this by developing community relationships and maintaining positive relationships with customers and referral sources within the Community. The successful HL will quickly respond to customer and community needs and describe the Medicare Hospice Conditions of Participation to increase awareness of available services and program requirements. This position develops, facilitates and coordinates the referral process to promote a seamless, timely and comfortable transition experience for referring providers, patients and their families. The Hospice Liaison frequently provides presentations of hospice philosophy and program requirements to an array of healthcare and community audiences.
REQUIRED EDUCATION & EXPERIENCE:
• Must possess one (1) year of sales, marketing, professional relations and/or education training required (healthcare background preferred)
• Bachelor’s degree in a related field preferred
• Hospice experience a plus
• Must have a high school diploma or equivalent
• Must be at least 18 years of age
REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
• Must possess the ability to express with care and concern the hospice care philosophy and goals
• Must possess the ability to communicate with the general public and represent Bristol in a positive manner
• Must demonstrate an understanding of, and commitment to comply with, all applicable health care laws and regulations
• Must possess the ability to articulate the Hospice Conditions of Participation and facilitate the transitions in care for hospice patients, family members, caregivers and other members of the healthcare continuum
• Must possess the ability to distinguish between the HL role which does not involve clinical determinations and the role of the Bristol clinicians who have the sole authority to admit patients to hospice service
• Must possess the ability to create relationships with referral sources, patients and families
• Must possess the ability to manage time and provider contacts effectively to achieve desired outcomes
• Must possess the ability to work unsupervised, exercising a high degree of integrity, discretion, independent judgment, self-motivation and resourcefulnessOTHER REQUIREMENTS:
• Must have excellent verbal and written communication skills, problem solving skills and basic math skills
• Must be flexible in scheduling, including evenings and weekends, to ensure that patients and their families need’s for hospice services can be met upon their timetables and schedules
• Must be willing to travel throughout Bristol’s hospice service areas
• Must have excellent personal hygiene and professional attire as this position represents the company throughout the community
ESSENTIAL JOB FUNCTIONS:
• Comply with all applicable state, federal and local government laws and regulations relations as well as Bristol policies, procedures and standard practice including but not limited to those related to Fraud & Abuse and Anti-Kickback
• Facilitate initiatives that promote hospice awareness and access in local communities
• Ensure the continual growth of Bristol Hospice through advocacy with referral sources including, but not limited to, hospitals, physicians, nursing homes, ALFs and other community partners
• Ensure the educational needs of the community related to the availability of and COP requirements for hospice care and services are assessed and met on an ongoing basis
• Continuously conduct community education needs assessments and develop plans designed to meet community services as determined by those assessments
• Establish and monitor community, customer, payor and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community
• Facilitate communication between Bristol clinicians and referral sources, as needed, to promote satisfaction and facilitate the continuum of care
• Maintain comprehensive working knowledge in the field of community educational resources and share information with appropriate organizational personnel
• Present comprehensive hospice information to multiple locations, hospice patients, family members, caregivers, hospitals, physicians, assisted living facilities and other environments
• Serve as liaison between facility, physicians, hospitals, skilled nursing facilities, community and hospice programs, by insuring prompt and accurate flow of information to Bristol clinical admission team for admission process and services
• Coordinate and communicate with hospice agency leadership, hospice clinical team and referral sources to promote effective care coordination and appropriate admission eligibility processes
• Promptly develop and maintain relationships of trust and integrity within the community
• Promptly respond to customer and referral source requests and concerns
• Serve as a hospice resource to the community and facilities to promote the continuum of care
• Provide in-service trainings and educational programs to the community and facilities on Hospice COPs and Bristol service options
• Coordinate with the Bristol Executive Director and Director of Patient Care Services regarding the planning, implementation and evaluation of in-service and continuing education programs
• Provide required documentation as it pertains to the HL role• Collect and monitor data of referral/admission patterns from all sources including, but not limited to, physicians, social workers, discharge planners, hospitals, ALFs and nursing homes
• Recommend hospice advocacy educational plans or revisions to educational plans for implementation in the professional referral community in conjunction with the Executive Director and the EVP of Community Development
• Ensure patient’s rights and confidentiality are always protected
• Attend mandatory meetings as required
• Comply with accepted professional standards of excellence, ethics and principles
WORK ENVIRONMENT:
• Must comply with company policies, procedures and processes in addition to Hospice rules and regulations
• Must be dependable and flexible in work schedule
• Must comply with harassment and discrimination policies
• Must foster good working relationships
• Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
• Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter
PHYSICAL ENVIRONMENT:
• Walking/mobility, reaching, bending, lifting, grasping, pushing, pulling and fine hand coordination
• Must be able to perform prolonged or considerable walking or standing
• Must be able to lift up to 50 lbs. without assistance
• Must be able to see and hear with or without accommodations
• Potential exposure to occupational hazards including, but not limited to, infectious/contagious diseases, heat, cold, noise, hazardous substances and uncontrolled home environments
