Our community-based services provide personalized one-to-one healthcare in the comfort of their homes, with a special emphasis on comfort, respect, kindness, and compassion. We offer a Continuum of Care that is never-ending, ensuring that our patients and families receive the physical, emotional, social, and spiritual support they need.
We are currently hiring for a Per Diem RN Case Manager to join our team immediately. This position qualifies for a sign-on bonus, and the pay-out is based on the average number of hours worked. While prior hospice experience is great, we are more interested in hiring a great RN with a passion for hospice care.
As a member of our team, you will work in a focused territory in King County, although you may be asked to change territories or cover other areas from time to time. You will be based out of our offices in Georgetown, where we hold trainings, meetings, and other team activities.
Your key responsibilities will include conducting comprehensive assessments, developing care plans in collaboration with patients and their families, case management, education, coordinating resources, delegating to other team members, pain and symptom management, and skilled medical interventions. You will work closely with patients and their caregiving team to maintain regulatory compliance, and honor the patient and family with compassion and care at this pivotal time.
To join our team, you will need an RN degree from an accredited school, a current RN license, a valid driver's license, auto insurance, and a dependable vehicle. Prior experience in hospice, oncology, or home health is great, but we are more interested in hiring a great RN with a passion and interest in hospice care.
