- Provide hospice medical services to patients
- Participate in certifying and recertifying patient eligibility for all levels of hospice care including GIP admission, with responsibility for carrying the community phone (admissions) and responding to eligibility questions in a timely manner and according to company guidelines.
- Coordinate medical care to assure competent, appropriate delivery of hospice services.
- Provide in-person and telehealth visits for patients in all care settings to assess patients’ condition and needs, review and make changes to plan of care (POC), and write appropriate physician orders.
- Act as the hospice attending physician when requested in the absence of other attending physicians, or for traveling patients when an alternative attending physician cannot be identified.
- Communicate information to and collaborate with attending physician regarding assessment review of changes to the POC and physician orders.
- Provide expertise and consultation in the delivery of pain and symptom management.
- Complete administrative and clinical documentation in a timely manner and according to company guidelines.
- Participate in on-call duties: PM call and weekend call.
- Participates as an active member of the Interdisciplinary Team
- Interact effectively in a team environment made up of other health care professionals, paraprofessionals, and volunteers.
- Participate in the interdisciplinary development, evaluation, and revision of the plan of care.
- Collaborate with interdisciplinary team members and attending physicians in resolving care conflicts and ethical dilemmas.
- Serves as an Agrace liaison and ambassador
- Promote hospice care and services in the community by acting as a liaison with community providers, facilities, medical schools, and teaching hospitals.
- Contribute to medical education at Agrace including direct/indirect supervision of learners and offering input to the UW Hospice and Palliative Medicine Fellowship hospice curriculum.
- Assist in the development of policies, procedures, forms, and service programs.
- Participate in departmental and/or agency-wide quality improvement activities.
- Precept new medical services clinicians during their onboarding process.
- Provide consultation to patients in the Palliative Care program
- Participates in palliative consults as a resource to the nurse practitioner.
- Remains current in evidence-based palliative practice.
- Doctor of Medicine or Osteopathy.
- Current unrestricted MD or DO Wisconsin state medical license.
- Board Certification in Hospice and Palliative Medicine, Hospice Medical Director Certification, or equivalent experience.
- Current unrestricted Wisconsin DEA registration.

How You’ll Make a Difference (Essential Functions)
The Hospice Physician provides medical services to hospice patients in all settings: home, facilities, hospitals and our Inpatient Unit. The physician is a key member of the interdisciplinary team, and partners with other clinical staff to coordinate hospice care for our patients and their families that is consistent with the goals, definition, philosophy and purpose of Agrace.
