      Job summary

      We are seeking a dedicated and experienced post holder to join the Hospital and Care at Home (H&C@H) team.

      The post holder will work within the multidisciplinary team to deliver high-quality care, providing clinical and managerial leadership in the absence of the clinical lead. You will organise the care environment to meet patient needs, improve their experience, and support staff in achieving divisional and Trust objectives.

      Key Responsibilities:

      • Provide expert clinical assessments and deliver high-quality care within established protocols
      • Utilise evidence-based clinical skills and digital technology to care for patients in their own homes
      • Manage patient caseloads, ensuring effective handover of care and timely communication
      • Develop and maintain seamless referral pathways to other agencies and specialist teams via the multidisciplinary team (MDT)
      • Work within your scope of practice and clinical competencies, ensuring compliance with professional policies, national and local guidelines, and Trust procedures
      • Play a vital role in delivering and developing formal and informal education for MDT members
      • Offer specialist advice and support through advanced clinical reasoning and evidence-based practices
      • Contribute to service development and improvement initiatives.
      • Work autonomously, assessing patient needs and making informed clinical decision

      About the Role:

      As an Autonomous Practitioner, you will develop and maintain a highly specialised knowledge base and clinical assessment skills, working within established protocols. Your primary responsibility will be to deliver, monitor, and escalate care for patients on the H&C@H virtual ward. You will liaise with medical colleagues and multidisciplinary teams to assess, implement, and modify treatment plans, ensuring timely reassessment of patients.
