Job Description
- Req#: 529461
- Regularly meet with clients in their homes and other locations as needed throughout the community.
- Establish supportive and motivational relationships with clients while providing HSSS, HSS, and ICS services.
- Complete onboarding process for new clients, including assessments needed for HSSS, HSS, and ICS services.
- Connect clients to community resources and programs.
- Maintain client records in electronic health record system (Credible).
- Collaborate with other family members, service providers, staff members, consultants, interns and/or volunteers.
- Valid driver's license, vehicle insurance, and acceptable driving record required
- Experience providing Housing Support Supplemental Services is highly desired
- Experience providing Housing Stabilization Services is highly desired
- Experience providing Integrated Community Services is highly desired
- Experience working with individuals who have experienced long-term homelessness is desired
- Experience working with individuals affected by mental health and/or chemical dependency issues is desired desired
- Bachelor's degree in a behavioral health field preferred
