      ReDiscover is seeking a Housing Outreach Liaison to add to our Transitional Living Program !

      ReDiscover has been known for over 50 years as a non-profit community mental health center in southeastern Jackson County, Missouri with a mission of delivering efficient and cost-effective treatment for the people we serve.       WHAT WE DO

      Our Transitional Living Program is designed to inspire and motivate young adults to accomplish their individualized independent living hopes and dreams. With the use of life coaches and community support specialists, they are able to navigate living independently in their own one bedroom apartment within the community. We put an emphasis on the young adults' strengths and what their success looks like long term. We provide intensive daily support, case management, life coaching, medication management, resources & entitlements, employment & educational supports, in a client centered strengths based supportive atmosphere.

      At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:

      • Outreaches to individuals not currently engaged in services who are in a housing crisis or experiencing homelessness.
      • Refers and transport individuals, as needed, to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits.
      • Attends local Continuum of Care meetings related to coordinated entry/case conferencing.
      • Receives referrals from Continuum of Care programs to assist individuals with housing stability.
      • Recruits landlords and build rapport by directly contacting potential landlords to provide information and make referrals to their units. Follow up with landlords about referrals and problems.
      • Safeguards Protected Health Information (written and electronically) and maintains confidentiality. This may include providing written documentation, scanning and/or uploading records into the Electronic Medical Record (EMR) system. This may also include requesting and/or releasing client information or medical records.


      We are focused on your development and will spend time helping you continue to grow as a Community Support Specialist. However, here are some that you will need prior to starting with ReDiscover:

      • 4-year degree in a social science field or 4 years of equivalent experience in lieu of a degree
      • A valid automobile driver’s license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work
      • Working knowledge of mental illness and substance use disorders
      • Excellent customer service skills with a recovery focus
      • Strong written, oral, and interpersonal communication skills
      • Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite
